Community Health Nurse

Administration – P/T - .30 FTE

Provide nursing care to families in their own homes, and/or in community settings such as social services agencies, shelters and/or transitional housing programs. 

Essential Duties and Responsibilities

  • Provide nursing assessment, nursing diagnosis and treatment, including immunizations, injections and health screenings, to children and adults per clinic and community standards.
  • Provide community and primary care referrals to children and adults as appropriate.
  • Under supervision of a primary care, maintain program documentation and patient charting, including patient encounter forms and progress notes for each patient served.
  • Develop and maintain effective communications and productive working relationships with internal and external teams.
  • Assess, treat, and document treatment of any minor illnesses in patients for whom clinic visits would be difficult, or unnecessary. Triage to outpatient clinic care as necessary.
  • Demonstrate a commitment to the mission, core values and goals of HealthPoint and its healthcare delivery including the ability to integrate values of justice, respect, compassion, excellence and stewardship into appropriate programs and services.
  • Perform other duties as assigned by supervisor. 

Secondary Duties and Responsibilities

  • Follow nursing procedures and charting protocols to screen children for immunization, health or developmental needs or problems.
  • Receive, assess, document and triage calls from community agency staff. Understand, enhance and give input into modifying triage system
  • Prepare and submit reports summarizing HealthPoint’s community-based services as required by funders.
  • Provide education and in-services for community agency and HealthPoint staff concerning health needs of children and families.
  • Assist community agency staff in controlling communicable diseases on site.
  • Serve as liaison for HealthPoint to other health and human service agencies who serve the same patients. 

Education and/or Experience 

  • Bachelor’s degree (BSN) from four-year college or university and three to five years related experience; or
  • equivalent combination of education and education. 

Licenses or Certificates 

  • WA State RN License
