Yes, you could be eligible for FREE insurance. Stop by HealthPoint Midway on Saturday, December 13 from 11:00am-3:00pm to speak with our certified patient navigators. They can find out if you qualify and sign you up on the the spot! If you answer 'yes' to these questions, then come see us:
- Are you a US citizen, legal resident or qualified immigrant?
- Are you 18-64 years of age, and
- Not currently insured by Medicare?
There are also free and low cost health insurance programs for children. Talk to us about getting all of your family insured.
You will need to bring identification (driver's license, naturalization certificate, permanent resident card and/or passport), proof of income (tax return, pay stub and employer information) as well as provide social security number and address.
