Patient-Centered Medical Home
We are proud of our Patient Centered Medical Home (PCMH) certification from the National Center for Quality Assurance. As a PCMH our patients depend on an integrated team and are active members in their personal journey to good health. Our integrated teams include, primary care providers, Behavioral Health Providers, Natural Medicine providers and they all work together to ensure that our patients receive the right care, in the right amount, at the right time.
