Do You Need Help Paying for your Care?

Everyone can be a HealthPoint patient. We are here to help you get the health care you need, no matter what your circumstances.

If you are unable to get insurance, we may be able to offer a sliding fee scale which is based household gross income and family size. Our sliding scale fees start at $25 for a medical appointment and $50 for a dental appointment.

In order to qualify you will need to bring proof of income for the last 30 days. Provide copies of one of the items from the list below unless you have multiple sources of income for people in the household who are 18 and over:

Examples of Proof of Income

- Your most recent 1040 tax return.

- Two recent pay checks that show year to date income.

- A recent letter from an agency that gives you support, like the Department of Social and Health Services (DSHS), Social Security, or the Employment Security Department.

- One month of current pay stubs.

- Current wage statements (written statement from employer.

- One month of current unemployment check stubs.

- Current statement from Social Security office.

- Letter from the individual who supports the patient financially. The letter must state a dollar amount. Not valid for spouses.

- First page of current or previous year income tax forms.

- Previous year W-2 form.

- For business owners, current or previous year profit/loss statement, line 7, on tax form.

You will not have to pay out of pocket for your care. Our staff can help you apply for insurance or other programs that will cover any associated costs.

How long after my appointment do I have to turn in proof of income to apply for Sliding Scale?

You will have 60 days to turn in documentation.

Do I have to have insurance to apply?

No. You may apply whether you are insured, underinsured, or uninsured.

Will applying affect my credit or immigration status?

No. Applying for financial assistance will not affect your credit or immigration status.

What if I do not have all the required documents?

Please submit what you have. Our team can help you understand what is still needed.

How long does financial assistance last?

The length of financial assistance depends on the specific program. Some programs provide coverage for 12 months, others last until grant funding ends, and some apply only to past balances. Please check with a fron desk representative for details about program length(s).

Can I apply after I receive a bill?

Yes. You may apply even if you have already received a statement.
