The Patient and Family Advisory Committee is a volunteer group of HealthPoint patients, family members, patient care takers, and community partners. The goals of the Committee are to:

  • Improve the patient and family experience at HealthPoint
  • Support our communities
  • Build strong relationships with each other
  • Find solutions together to improve health and wellbeing

PURPOSE:
The Patient & Family Advisory Committee (PFAC) will have an active role in improving the patient and family care experience at HealthPoint and strenghtenting our communitues.

PFAC RESPONSIBILITIES:

  • Attend meetings on a regular basis
  • Serve in an advisory capacity to HealthPoint, not as a decision-making body
  • Discuss patient, patient family/caretaker, and community concerns/needs
  • Act and speak with the goal of improving patient and family members’ care
  • Prepare for meetings and discussions by reading materials sent before meetings
  • Provide feedback, both positive and negative, in a constructive way
  • Abide by the Code of Conduct
  • Be willing to serve on PFAC for a minimum of one year

MEMBER SELECTION:

  • A patient, family member, or patient caretaker may join independently or be referred to join by a HealthPoint staff member. For more information, please contact pfac@healthpointchc.org
